Wellness Coach

Axis Health System is seeking a full-time Wellness Coach at its Pagosa Springs Integrated Health clinic in Pagosa Springs, CO. The Wellness Coach supports patient wellness and health integration in outpatient clinics. Wellness coaches are an integral part of a patient's Care Team and work within multidisciplinary teams and other support staff. This position is responsible for engagement of clinic patients into a variety of healthcare activities through population-based assessments and screenings and tracking patients' follow-up and progress. Wellness coaches focus on prevention, health education, brief interventions and referral to additional care when indicated.

Qualifications: Our ideal candidate for our Wellness Coach position will have their high school degree (or equivalent). A bachelor's degree in social work, Psychology, Counseling, or equivalent field is preferred. Two years of volunteer or paid experience in a healthcare or social service setting is also preferred.

Required Skills: Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax, and multi-line telephone. Working knowledge of Windows-based computer applications including, MS Edge, Excel, Outlook, Teams, Word, Office 365, and ability to access the internet. Ability to be helpful and sensitive to the needs of all employees, while concurrently respecting corporate policies, procedures, and priorities. Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers. Ability to communicate job-related information to individuals and small groups. Ability to work independently with minimal supervision; plan own work schedule effectively, multi-task and prioritize in a fast-paced setting. Ability to multi-task, organize and prioritize workload in a sometimes-hectic environment with frequent interruptions. Ability to concentrate in sometimes hectic and stressful settings. Ability to read and understand written materials. Must regard work as confidential. Must have current driver's license, qualify for commercial vehicle insurance and maintain a driving record that allows insurability with Axis' insurance agency. Travel between clinics as required. Ability to take on additional duties when necessary.

Additional Eligibility Requirements: Annual Flu immunization, Annual TB screening, CPI training within 90 days of hire, BLS PRO certification required within 90 days of hire.

Salary: Starting pay is $18.50 (Based pay $17.50 + $1.00 Locale Differential = $18.50) and ranges up to $22.05 per hour, which includes a $1.00 locale differential at our Pagosa Springs worksites and considers preferred experience and preferred education described above.
